Referral Requiments

At this time, we accept new patients with a referral only. This policy is in place to ensure we provide the best care possible to our patients. We accept referrals from both primary care physicians and specialists.

Exceptions include patients self-referring for ILIT or SLIT therapies.

Referral guidelines:

  • All referrals should include a referral diagnosis, demographics page, insurance information, and recent office visit notes pertaining to the referral. Please send applicable imaging, testing, and bloodwork. 
  • Referrals lacking the above information will be sent back to the referring office for more information.
  • If a referral is truly urgent, we welcome phone calls to discuss specific cases in person.
  • If patient has previously seen another allergist, please send those records as well, or ask that your patient have them sent prior to their visit
  • If you are referring a patient for environmental allergies, please refrain from ordering serum testing prior to the visit. This may keep us from performing skin testing, which is both more sensitive and comprehensive, as insurance may not pay for both.
  • We will fax the referring office when referrals are received/processed and when patients are scheduled

 

For patients:

  • We receive a large volume of referrals each day. While we make every effort to process referrals within 2-3 business days, there may sometimes be a delay, especially if we don't receive all required information. 
  • If we are not in network with your plan, we will notify the referring office so that they may refer you elsewhere.
  • Once your referral is processed, we will call you to schedule a total of three times. If we are unable to you reach you, we will stop trying to call you to schedule, but you may call us to schedule.
  • Please do not call our office to see if we have received a referral. If you have not received a call from our office within 1-2 weeks, please check with your referring office to make sure the referral was sent. They may check with us to see if the referral was received.
  • Bringing previous allergy records to your appointment greatly helps us provide you better care.
